While operating of Okinawa, O'Brien (Cdr. William Woddward Outerbridge) in heavily damaged by a Japanese kamikaze aircraft in position 2616'N, 12726'E.  53 Of the crew were killed and 73 wounded and amongst them the Commanding officer Cdr. Outerbridge.

Stricken 18 February 1972.
Sunk as a target off the coast of California 13 July 1972.
